Why are my bowel movements painful and sometimes have blood?

While making a bowel movement I feel pain and I usually bleed. Its not a lot a blood, it's only a little on the paper or within my stool. This is usually everytime I go number 2. I don't feel any other pain when im not going its only when im actually going to the restroom. I've watched doctor Oz....(which may or may not be a good source) but from my understanding my stool is not suppose to be in small clumps on pebbles. However, my stool is small clumps or even pebble like. Its been this way for as long as I can remember. Is there something wrong?
